Upgrade to ACUVUE® OASYS 1-Day for ASTIGMATISM. CLRS Cashback Offer

By participating, all claimants will be deemed to have accepted and be bound by the terms and conditions. Please retain a copy for your information.
Promoter: Johnson and Johnson Vision, Johnson & Johnson Medical Ltd, Pinewood Campus, Nine Mile Ride, Wokingham, RG40 3EW.

Eligibility:

  1. Offer applies to UK (England, Scotland, Wales, Northern Ireland, the Isle of Man and the Channel Islands) residents only, aged 16 and over holding a valid optical prescription dated within the 2 years prior to the date of the qualifying purchase. The promotion excludes employees of the Promoter and its subsidiaries, their immediate families, agents of the Promoter or anyone else professionally connected to the promotion.

  2. Entrants aged 16 or 17 must have parental/guardian permission prior to entering.

  3. [UK resident parents/guardians of children under 16 may claim the refund in respect of any such children that are switched to ACUVUE® OASYS 1-Day for ASTIGMATISM on the parent’s/guardian’s Contact Lens Reward Scheme account. In such circumstances, it will be the responsibility of the parent/guardian to comply with the terms and conditions below and/or procure that the child complies with the terms below to the extent they are relevant. The children under 16 must be UK (England, Scotland, Wales, Northern Ireland, the Isle of Man and the Channel Islands) residents holding a valid optical prescription dated within the 2 years prior to the date of the qualifying purchase. ]

Offer only applies to 1-DAY ACUVUE® MOIST for ASTIGMATISM patients wearing any astigmatic prescription between -6.00 to -9.00 power range.

Promotional Period:

Offer closes at midnight on 31.10.2019. All claims must be received by midnight on 14.11.2019

Background

  1. You have agreed to upgrade from 1-DAY ACUVUE® MOIST for ASTIGMATISM to ACUVUE® OASYS 1-Day for ASTIGMATISM. You will then receive a cash back cheque of £36. You can claim the cash back as soon as you have upgraded to ACUVUE® OASYS 1-Day for ASTIGMATISM on the Contact Lens Reward Scheme.

  2. If you wish to keep using ACUVUE® OASYS 1-Day for ASTIGMATISM, you do not need to do anything. Your Direct Debit charge will remain at the higher amount applicable for ACUVUE® OASYS 1-Day for ASTIGMATISM and you will continue to receive ACUVUE® OASYS 1-Day for ASTIGMATISM.

  3. If you wish to go back to 1-DAY ACUVUE® MOIST for ASTIGMATISM after 31.10.19, you must notify your optician before 31.10.19. Your optician will contact you in advance of 31.10.19 to ask if you would like to go back to 1-DAY ACUVUE® MOIST for ASTIGMATISM.

How to claim your £36 cash back cheque:

  1. To claim the cash back of £36 in cheque form you must upgrade to ACUVUE® OASYS 1-Day for ASTIGMATISM on the Boots Opticians Contact Lens Rewards Scheme from any participating store in the UK during the Promotional Period.

  2. Entrants must have a contact lens consultation at a participating optician prior to purchase. Following a valid purchase, you may fill in the application form provided in store and return it, stamped by your optician confirming the switch to ACUVUE® OASYS 1-Day for ASTIGMATISM from 1-DAY ACUVUE® MOIST for ASTIGMATISM. Ask your optician to provide and stamp an application form confirming the purchase and send the original completed and stamped form to: Upgrade to ACUVUE® OASYS 1-Day for ASTIGMATISM.   3. Where the Promoter receives a valid claim from you, you will be entitled to £36 cash back. No alternative reward will be provided.

  4. The £36 cash back will be sent in the post to you in the form of a cheque made payable to the individual listed on the application form within 28 days of receipt of a valid claim.

 

General:

  1. You are solely responsible for ensuring that you cash any cheque before its expiry date. The Promoter will not re-issue a cheque in the event that you fail to cash it before it expires, or for any other reason. The Promoter cannot be held responsible in the event that any cheque is not received by you as a result of you having provided an incorrect or incomplete address on the application form.

  2. Only one claim per person [(save that parents/guardians of children under 16 years may claim a cash back in respect of any such children that meet the entry requirements in addition to claiming on behalf of themselves in accordance with these terms)]. Anyone trying to circumvent this rule by using alternative details will be disqualified from this promotion. No bulk or third party entries will be permitted.

  3. This offer cannot be used in conjunction with any other money back offer.

  4. Claims that do not meet the specific entry requirements will not be processed. Cash back cheques can only be sent in the post by the Promoter and cannot be collected from any optician or in-store. Any illegible, damaged or incomplete claims will be deemed invalid and be rejected.

  5. Proof of submitting an application form is not proof of delivery to the Promoter. The Promoter is not responsible for applications that have been lost, delayed, mislaid or damaged in the post.

  6. The Promoter reserves the right to amend or suspend the promotion or these terms and conditions in the event of exceptional circumstances beyond its reasonable control or where fraud or abuse is suspected, but will always try to minimise the effect on participants in order to avoid disappointment.

  7. The Promoter reserves the right to verify all applications and to refuse to award claims where there are reasonable grounds to believe there has been a breach of these terms and conditions or any instructions forming part of this promotion’s requirements.

  8. We will use your personal data to administer this offer and dispatch your refund cheque, in accordance with our privacy policy www.acuvue.co.uk/privacy-policy.  The data controller is Johnson & Johnson Medical Ltd.

  9. Any personal data relating to entrants, including your name and any address provided, will be used solely for the purposes of this promotion and will not be retained for any other purpose. Data may be provided to a third party supplier (Ashfield Meetings and Events) where appropriate for the purposes of fulfilling delivery of the refund cheque to claimants only and will not be disclosed to a third party for any other purpose without the individual’s prior consent. Participants can withdraw consent to processing of their personal data at any time by contacting the Promoter at [email protected] but the Promoter will not be able to send cash back cheques to anyone who withdraws their consent.

  10. This offer and these terms and conditions are governed by English Law. In the event of a dispute, you or we may bring a legal claim in the courts of England and Wales, or in your local courts if you are a resident of Scotland, Northern Ireland, the Isle of Man or the Channel Islands).
