How to take part
Buy a 3 months’ supply (90 pairs of daily disposable lenses or 6 pairs of reusable lenses), before 30 June 2010 and you can claim £15 of M&S, hmv, John Lewis, Debenhams or amazon.co.uk gift vouchers.
Plus, if you buy a second 3 months’ supply before 30 June 2010 you can also claim an additional £15 of gift vouchers.
This offer is open to new ACUVUE® customers only, so ask your optician for a trial and you’ll soon be rewarding your eyes and rewarding yourself with up to £30 of gift vouchers to spend.
Simple steps to claiming your reward
Redeem your vouchers by following these simple steps:
• Contact your nearest optician to arrange a contact lens appointment*
• Buy 3 months supply of any ACUVUE® contact lenses
• Ask you optician for a claim voucher leaflet with their section completed and signed
• Simply collect the ends from 6 packs of any ACUVUE® daily disposable lenses or 2 pack ends from any ACUVUE® reusable lenses.
• Send these pack ends with your completed claim form back to us, and we will send you £15 of your chosen gift vouchers from: M&S, hmv, John Lewis, amazon.co.uk or Debenhams
REMEMBER if you buy a second 3 months’ supply before 30 June 2010 you can also claim an additional £15 of gift vouchers.

Terms and Conditions:
1. Your optician will need to make sure your eyes are suitable for contact lenses and there may be a charge for an eye examination and contact lens assessment.
2. Offer is only valid at participating opticians and is only open to UK residents aged 16 or over who have not previously worn ACUVUE® Brand Contact Lenses and who purchase a minimum of 3 months’ supply of any ACUVUE® product during the offer period for the first time. Offer excludes employees of Johnson & Johnson Vision Care.
3. The offer period: 1 January 2010 to 30 June 2010.
4. To be valid all claims must be made by 14 July 2010.
5. There is a maximum £30 worth of gift vouchers allowed per person and a maximum of 2 people claiming per household.
6. All claims must be accompanied by the official claim form and the appropriate number of ACUVUE® pack ends.
7. The first claim must include the section completed and signed by your Eye Care Professional (optician) and the second claim form must have the same unique reference number as the first claim form.
8. First claim forms not completed and signed by your Eye Care Professional (optician) will be rejected.
9. Claim forms with inadequate address details, that are incomplete, damaged, photocopied, faked, illegible or non matching will be rejected and no gift vouchers will be issued.
10. Allow 28 days for delivery of your gift vouchers.
11. No cash option and no alternative reward will be offered.
12. Vouchers may be delivered in the form of gift cards and are subject to availability. The organisers reserve the right to substitute vouchers where supply is affected.
13. Vouchers and gift cards are mailed by second class post (amazon.co.uk gift vouchers will be sent by email). Lost vouchers cannot be replaced under normal circumstances.
14. Promoter cannot be responsible for applications lost, delayed or damaged in the post.
15. The personal information and data collected by Johnson & Johnson Vision Care will ONLY be used for the operational requirements of this promotion and will only be shared with the third party working on our behalf for this purpose and for no other purpose and to no other third party.
